Pay is dependent on applicant's relevant experienceSummary:Organizes and coordinates the activities and flow of work for the transition of complex Care patients.
Reduces the overall Length of Stay, decrease readmission, and ensure a safe disposition for patients.Responsibilities:Assesses, plans, implements, coordinates, monitors and evaluates the options and services required to successfully transition patients with complex care needs and barriers to discharge from the hospital to the community.Utilizes appropriate resources to address complex care needs which may include, but not limited to: guardianship, foreign nationals and bariatric patients with significant post-acute needs, history of behavioral or substance abuse requiring specialized placement, frequent readmissions and lack of housing.Acts as a resource to the Social Work and Care Management teams as it pertains to complex patients.
Providing mentorship and guidance around skilled communication, education and resources that promotes quality and cost-effective outcomes as it pertains to complex patient care situations.Collaborates with managers to develop innovative options for complex patients.
May participate in leadership meetings and planning sessions regarding complex care patients.
Subject matter expert for the transition of complex care patients.Within scope of job, requires critical thinking skills, decisive judgement and the ability to work with minimal supervision.
Must be able to work in a fast-paced environment and take appropriate action.Requirements: Master's degree in Social Work.
